Patients must sign this form before receiving text messages. This consent process clarifies that mobile numbers are used exclusively for administrative communication, message frequency may vary, message/data rates may apply, and patients may opt out at any time by replying STOP or request assistance by replying HELP. Outgoing text messages may be used for appointment reminders, new appointment offers, billing updates, instructions for telehealth visits, or to notify patients that forms or documents are available. Mobile numbers, SMS opt-in data, and communication records are never sold, shared, or used for marketing. No clinical content is ever sent via text message.

If Synergy Behavioral cannot reach an individual, voicemail may be left with the practice name, callback request, and contact number; clinical information will not be included unless the patient has provided written authorization.
